Your doctor visits focus on diagnosis and treatment. Your provider wants to ensure you have support between visits, where most health management actually happens. This program helps you manage your health day-to-day between those visits.

Typically, you'll receive at least one check-in per month, with additional support available when needed.

No, only one provider may bill for CCM services each month so you don't enroll with multiple doctors. However, one of the benefits patients find very helpful is that we will coordinate appointments with other providers as needed on your behalf, even if outside our practice group.

This program is covered by Medicare. You may have a small monthly copay, depending on your insurance, coinsurance, or deductible obligations, similar to what you may be paying for a standard office visit. Your insurance will be billed for these services. Some patients do not have a co-pay at all. You can reach out to our office to confirm your co-pay.

No — this program supports your doctor and their care plan for you, it does not replace them.

Yes. You can opt out of the program at any time. These services are voluntary. You may unenroll at any time, effective at the end of the month in which you notify the practice.
